Need to Verify an Employee’s Credential?

If you are an employer/agency and wish to verify a practitioner’s certification, please complete the following form.

Email notifications will be sent to the email address specified.

*All credential holders have pdf certificates for verification.  If you need official verification from our office, please see below.

There is a $25 fee for verification.  We will confirm the credential requested. [Free for Military/Veteran request]

Please note: the NBCOS will verify the following: OPE-C, OPE-SC, ROT-SC.

We do NOT verify the following: OTC, Surgical Assist, Nursing, Occupational or Physical Therapy

Please follow the steps below:

1. If you are requesting one of the above credentials listed, please CLICK HERE to process $25 fee

2. Once payment is complete, fill in the form to the right with all required details

3. NBCOS Support Team will confirm credential to be verified via email with all the details requested (i.e., name, certification date, expiration date etc) within 2-3 business days.

4. If you have a document to be completed, please send via email to support@nbcos.org .
